Our care, where you live

Our live-in care in Somerset

Live-in care means one-to-one support in your own home: personal care, medication support, meals, help around the house, getting out and about, and someone close by at night. Whether it suits the person, and whether one carer can support a couple, is worked out at the free assessment. Read more in live-in care explained and how it works day to day.

How we arrange live-in care in Somerset

Day and Nightcare provides fully managed live-in care across Somerset, North Somerset and Bath and North East Somerset, from £1,550 a week. One main carer lives in the home; a relief carer covers their holidays and time off, and a manager is on call. Care starts with a free assessment, a written quote and a written care plan.

Care can be paid for privately, by the council (including direct payments) or by NHS Continuing Healthcare. We have provided Discharge to Assess (D2A) care since 2014. If someone is in hospital, tell us the expected discharge date (see care after hospital). Our care team is on 01865 715780.

Arranging care and funding in Somerset

First, check which council. Three unitary councils cover the area on this page. Each runs adult social care and collects council tax, so the council tax bill shows which is your relative’s. Somerset Council has covered most of the county since 1 April 2023.

  • Somerset Council (most of the county, including Taunton, Yeovil, Bridgwater and Frome): 0300 123 2224 (Monday to Friday, 8.30am to 5pm), or the online Public Referral to Adult Social Care form, for yourself or someone else. To speak to a social worker out of hours, call 0300 123 23 27. You can also talk to someone from Adult Social Care at a Talking Café.
  • North Somerset Council (Weston-super-Mare, Clevedon, Portishead, Nailsea): Care Connect on 01275 888 801 (Monday to Friday, 8am to 6pm) or careconnect@n-somerset.gov.uk, or the online self-referral form.
  • Bath and North East Somerset Council (Bath, Keynsham, Midsomer Norton, Radstock): 01225 394570 (Monday to Thursday 8.30am to 5pm, Friday 8.30am to 4.30pm). The first call is an initial assessment over the phone, which can take up to 30 minutes.

Out of hours, North Somerset and Bath and North East Somerset both use the Emergency Duty Team on 01454 615165. The needs assessment is free, and anyone can ask for one. Carers’ assessments go through the same council numbers (in North Somerset, also an online form).

Each council assesses needs, then finances if it may pay. With more than £23,250 in savings your relative would usually pay the full cost; the home is not counted when care is at home (see council funding for care). Paying privately? See the questions below.

For council tax, ask about the discount or exemption for someone who is severely mentally impaired (North Somerset calls it a reduction for a severe cognitive disorder), help when a carer lives in the home, and the disabled band reduction.

Urgent care and leaving hospital in Somerset

  • A&E, open 24 hours: Musgrove Park Hospital in Taunton, Yeovil Hospital, and the Royal United Hospital (RUH) in Bath.
  • Weston General Hospital: its emergency department is not open overnight. The trust gives its hours as 8am to 10pm every day; the NHS website lists 8pm closing for adults on some days. The nearest 24-hour adult A&Es are Bristol Royal Infirmary and Southmead Hospital.
  • Urgent treatment centres at Bridgwater, Chard, Frome, West Mendip (Glastonbury), Minehead and Shepton Mallet community hospitals and at Yeovil Hospital, and minor injury units at Burnham-on-Sea, Clevedon and Paulton. None is an A&E. Hours vary and can change at short notice, so check the town page or NHS 111 first.

Leaving hospital:

  • Somerset Council area: ask the ward staff, who can arrange for someone from Adult Social Care to see you. Home First offers reablement at home or a short stay in a reablement unit. Home First is free for no longer than six weeks.
  • North Somerset: the council’s TRI team gives free short-term reablement and telecare, usually for up to six weeks, on referral from council staff, Sirona’s Discharge to Assess therapists or the hospitals’ Transfer of Care hubs.
  • RUH, Bath: discharge planning starts as soon as someone is admitted, and the RUH asks patients to plan from then too; the hospital asks patients to arrange to leave by 10am; hospital transport is only for clinical need. The Community Wellbeing Hub’s team in the RUH Atrium (01225 821019) can help with support at home.

Three NHS boards arrange NHS Continuing Healthcare. NHS Somerset (the Somerset Council area): 01935 38 5233, option 1 (option 4 for Fast Track). NHS Bristol, North Somerset and South Gloucestershire (North Somerset): 0117 900 2626 or bnssg.chcteam@nhs.net. NHS Bath and North East Somerset, Swindon and Wiltshire: email bswicb.chcassessreview@nhs.net (no phone number is published).

NHS Continuing Healthcare

When someone's main need is a health need, the NHS pays for all of their care through NHS Continuing Healthcare, and it is not means-tested. It is arranged by the NHS board for the person's area: NHS Somerset Integrated Care Board for the Somerset Council area (Continuing Healthcare information); NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board for North Somerset; NHS Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board for Bath and North East Somerset. Your questions

Questions about live-in care in Somerset

Who do I call in Somerset to ask for a care assessment?

It depends on the council. Somerset Council (most of the county, including Taunton, Yeovil, Bridgwater, Frome and Minehead): 0300 123 2224, Monday to Friday 8.30am to 5pm. North Somerset Council (Weston-super-Mare, Clevedon, Portishead, Nailsea): Care Connect on 01275 888 801, Monday to Friday 8am to 6pm. Bath and North East Somerset Council: 01225 394570, Monday to Thursday 8.30am to 5pm and Friday to 4.30pm. Out of hours, Somerset Council's line is 0300 123 23 27, and North Somerset and Bath and North East Somerset share 01454 615165.

Can the council help if we pay for care ourselves?

Yes. Somerset Council asks people paying for their own care to contact it as soon as their savings drop to £30,000, and has a one-minute online financial eligibility checker. If your relative has more than £23,250 and asks the council to arrange care for eligible needs, it will normally charge an arrangement fee; the amount is not published. North Somerset Council's free Care Navigator service, for adults who pay for their own care, gives impartial advice by phone or a home visit, with lists of care agencies and advice on Attendance Allowance (ask Care Connect on 01275 888 801). You can also arrange live-in care directly with a provider.

Which hospice serves our part of Somerset?

St Margaret's Hospice Care gives free care across Somerset, mostly at home or in the community; its 24-hour Advice Line (01823 333822 or 01935 709480) is for anyone with a life-limiting illness registered with a Somerset GP, and you can call on someone's behalf. Dorothy House covers Bath and North East Somerset and parts of Somerset as far south as Shepton Mallet (0345 0130 555). Weston Hospicecare covers Clevedon to Highbridge and Cheddar, free (01934 423900). St Peter's Hospice in Bristol cares for people with a North Somerset GP, accepts self-referrals and has a 24-hour advice line (0117 915 9430).

Where is the nearest A&E that is open all night?

Musgrove Park Hospital in Taunton, Yeovil Hospital and the RUH in Bath have A&Es open 24 hours. Weston General Hospital's emergency department is not open overnight: the trust gives 8am to 10pm, and the NHS website lists 8pm closing for adults on some days. From North Somerset at night, the nearest 24-hour adult A&Es are Bristol Royal Infirmary and Southmead Hospital. In a life-threatening emergency, call 999; otherwise call NHS 111 for advice.

How much does live-in care cost in Somerset?

Live-in care with Day and Nightcare costs from £1,550 a week anywhere in Somerset, the same as everywhere in England, for one person with a carer who rests at night. That includes our care team's management (the assessment, care plan, reviews, supervision and the on-call manager) and the carer's food; break cover and the carer's travel are extra. For a couple it is from £1,800 a week, where one carer can safely meet both partners' needs. You receive a written quote after a free assessment.

Can the council help pay for live-in care in Somerset?

Yes, if the person's care needs meet the national eligibility criteria and their savings are below £23,250 (2026/27). Adult social care in Somerset is arranged by Somerset Council, North Somerset Council and Bath and North East Somerset Council. Ask for a care needs assessment; if you are eligible, council funding can usually be taken as a direct payment and used for live-in care from us.

Who arranges NHS Continuing Healthcare in Somerset?

NHS Continuing Healthcare is arranged by the NHS board for the person's area: NHS Somerset Integrated Care Board for the Somerset Council area; NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board for North Somerset; NHS Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board for Bath and North East Somerset. When someone's main need is a health need, the NHS pays for all of their care, including live-in care at home. Ask the GP, social worker or hospital discharge team for a CHC checklist assessment.

Can live-in care start when someone leaves Musgrove Park Hospital?

It can. Tell us the expected discharge date as early as you can, and we will work with you and the ward's discharge team on when live-in care can start. We can often start within 24 to 48 hours of the assessment, depending on carer availability, and we confirm the start date once a carer is arranged. We have provided Discharge to Assess (D2A) care since 2014.

How would live-in care be supported in Somerset?

Our care team supports every live-in placement, wherever it is in England: a free assessment, a written care plan, regular contact and reviews, one main live-in carer, with a relief carer covering holidays and time off, and a manager on call outside office hours. Your carer works alongside the person's own GP, community nurses and any hospice team already involved in their care.
